Why does a Lyrica 150 mg line name DPN, PHN, or fibromyalgia?

Last reviewed · 3 min read · Updated

Bone review card, cobalt edge, lime chip: one named indication on the card

Because the ceiling changes with the disease. Adult Lyrica starts for diabetic nerve pain, postherpetic neuralgia, fibromyalgia, and spinal-cord injury pain can all begin at 150 mg per day when kidneys are ordinary. DPN stops at 300 mg/day. PHN can climb through 300 toward 600 in selected adults. Fibromyalgia often lives at 300-450 and is not recommended above 450. Copying a neighbor's 450 onto a DPN chart is a labelled miss. Ceiling scan holds the numbers. Panel.

Why the disease has to sit on the line

US indications are neuropathic pain from diabetic peripheral neuropathy, postherpetic neuralgia, adjunct partial-onset seizures (age rules apply), fibromyalgia, and neuropathic pain from spinal cord injury. The 150 mg start is shared. The top is not.

A script that only says 'Lyrica 150 mg' without directions or a disease leaves the pharmacy guessing the split. Ask which row you are on.

What DPN does with 150 mg

Begin 50 mg three times daily (150 mg/day). The dose may rise to 300 mg/day within one week. Maximum recommended for DPN is 100 mg three times daily (300 mg/day) when CrCl is at least 60 mL/min.

600 mg/day was studied in DPN and did not add significant benefit; it was less well tolerated. Treatment above 300 mg/day is not recommended for that pain.

SCI pain and adult seizure adjunct

Spinal-cord injury pain starts at 75 mg twice daily (150 mg/day), may reach 300 mg/day within one week, and can go to 600 mg/day as 300 mg twice daily after 2 to 3 weeks at 300 if pain remains and the person tolerates it. That row is two divided doses.

Adult adjunct partial-onset seizures begin at 150 mg/day and may rise toward 600 mg/day in two or three doses. Pediatric dosing is weight-based and is not this adult 150 mg lock.

What PHN does with 150 mg

Begin 75 mg twice daily or 50 mg three times daily (150 mg/day). The dose may rise to 300 mg/day within one week. People who still hurt after 2 to 4 weeks at 300 and who tolerate the capsule may go toward 600 mg/day.

Reserve dosing above 300 mg/day for ongoing pain plus tolerance. Dose-dependent adverse reactions and higher dropout sit behind that reserve.

What fibromyalgia does with 150 mg

Begin 75 mg twice daily (150 mg/day). Rise to 150 mg twice daily (300 mg/day) within one week if needed. People who still lack benefit at 300 may go to 225 mg twice daily (450 mg/day).

600 mg/day was studied and did not add benefit; it was less well tolerated. Doses above 450 mg/day are not recommended for fibromyalgia.

Indication tag without mixing heights

Write the disease next to 150 mg. DPN 300. Fibro 450. PHN and SCI can see 600 in selected adults. Kidneys rewrite every ceiling. Kidney scan.

Board thread

Answers follow the U.S. Lyrica label.

Mina D., Olympia Is adjunct seizure the same 150 start as fibro?

Board reply

Adult seizure adjunct also begins at 150 mg/day. The ceiling can be 600 mg/day. Fibromyalgia is not recommended above 450. Same start, different top.

Ines F., Ann Arbor Neighbor swears 450 is when Lyrica works. I have DPN.

Board reply

DPN's labelled cap is 300 mg/day. Copying a fibromyalgia 450 onto a DPN chart is a miss. Ask which disease is on your line before you chase 450.

Cole B., Shreveport Script says Lyrica 150 mg, no disease. What do I ask?

Board reply

Ask which indication and which split. 150 mg can be the daily start or one capsule toward a 300 mg day. The pharmacy cannot guess the ceiling without the row.
