Pregabalin leaves mostly unchanged in urine. Mean half-life near 6.3 hours in healthy kidneys is short enough that one 150 mg swallow leaves a long afternoon trough and a peak that feels like a brick at night.
BID or TID exists to spread that day. Spinal-cord injury pain is written as two divided doses. Other adult rows allow two or three. Ask which split you were given.
When the day collapses toward once daily
CrCl 15-30 mL/min often uses once daily or twice daily at a smaller total. Below 15 mL/min the start column is once daily. Those rows are why a neighbor's BID card is not your card.
Hemodialysis removes a fraction. A supplemental dose after a four-hour run is labelled. Do not invent the extra milligrams from a group chat. Kidney scan.
A 150 mg capsule is not a 24-hour tablet
US strengths include a 150 mg capsule. People screenshot it and swallow it once because the number matches the daily start. With CrCl at or above 60, that gulp is usually the wrong shape.
If your written plan truly is once daily, a renal row or a specialist plan should be on the same paper. Ask. Do not guess.
When repeated 150 mg days sit still
After repeated doses, steady state is reached within 24 to 48 hours. Multiple-dose levels can be predicted from a single dose. That is a chemistry sentence, not a promise about foot pain.
People who change the hour every swallow never see a clean steady day. Pick morning and evening clocks and keep them. Missed-dose scan covers the skip rule.
Food does not set the BID hour
A meal is optional on the US line. It slows the peak (Tmax near 3 hours, Cmax down about 25 to 30 percent) without a clinically relevant change in total absorption.
That is not a reason to skip the evening capsule after a late dinner. Food scan owns the meal numbers. This page owns the split.
Cover without stretching the bottle
Do not stretch a 150 mg BID plan by taking one capsule 'to make it last.' Uneven days feel like withdrawal and look like misuse. Misuse flag.
