Where a GI switch is filed without a home crush
Gut noise sometimes eases after a prescribed IR-to-XR change. That visit is not a license to grind a neighbor's XR. The after-meal IR stair stays on GI titration.
Board-check: different NDC, swallow whole, evening XR, 2000 versus 2550. The 850 panel still locks IR.
When XR is usually swallowed
Adult Glucophage XR often starts 500 mg once daily with the evening meal and steps 500 mg weekly toward 2000 mg once daily. That clock is not the 850 IR breakfast-or-supper split.
If 2000 mg once daily XR is not enough, the label allows a trial of 1000 mg XR twice daily, or a switch to IR totals up to 2550 mg divided. Those are prescriber switches.
What 'not the same tablet' means on the vial
Release type is printed on the label. IR 850 mg and XR 500 or 750 mg are not interchangeable at the kitchen counter. A partner's XR bottle is not your missed 850.
Generic metformin without IR or XR on the receipt is a reason to read the vial, not a reason to assume this lock.
How 2000 mg XR and 2550 mg IR sit on different lines
IR ceiling 2550 mg per day in divided doses is the max scan. XR's usual daily ceiling is 2000 mg. Pasting 2550 onto an XR vial is a misread.
Patients on IR Glucophage may be switched to XR once daily at the same total daily dose, up to 2000 mg. That sentence is in Dosage. It is not a kitchen conversion.
Why XR must stay whole
Labelled XR directions say swallow the tablet whole. Never crush, cut, or chew it. Breaking the matrix dumps the day and is not a homemade 850 IR.
Some XR products leave a tablet-shaped shell in the stool. That ghost is a shell story, not proof the dose failed. Ask whether you were switched before you crush the next one.
