What is the boxed warning on metformin 850 mg?

Lactic acidosis. Postmarketing metformin cases have ended in death, hypothermia, hypotension, and resistant bradyarrhythmias. Onset is often subtle: malaise, myalgias, respiratory distress, somnolence, abdominal pain. Labs have included lactate above 5 mmol/L and metformin levels generally above 5 mcg/mL. If it is suspected, stop Glucophage 850 mg and treat in a hospital. This scan is that box. A normal eGFR is not a comment-thread reason to refuse the INN.

Where GI noise stops being 'usual' next to this box

New severe GI plus looking sick is not the early-diarrhea row. File usual first-week stool on the diarrhea scan and on the GI signal.

Board-check: box, subtle cluster, risk list, hospital. The 850 panel keeps a short emergency line. This scan is the warning itself.

What the boxed warning is naming

Metformin-associated lactic acidosis is the box. It is rare and still not a comment-thread wait. Glucophage 850 mg is an outpatient tablet for kidneys that work. It is not a hero drug in shock.

Anion-gap acidosis without ketones, a high lactate/pyruvate ratio, and an elevated metformin level are how the cases were characterized. You will not sort that at a kitchen table.

Which symptoms the box calls nonspecific

Malaise, muscle pain, trouble breathing, sleepiness, and belly pain are the named cluster. Cold hands, dizziness, or a slow irregular heartbeat show up in the Medication Guide list too.

Those symptoms also have common causes. The tag is: metformin on board plus this cluster plus a risk factor means do not wait for a forum vote.

Why a normal eGFR is not a reason to refuse the INN from a comment

People decline metformin because a thread said it 'causes acidosis.' In a person with eGFR 70 and a working liver, that comment is not the labelled risk picture.

The kidney cuts on the eGFR scan are how you stay out of the accumulation story. Fear of the box is not a fifth cut.

Which risk factors the warning lists

Renal impairment, certain drugs such as carbonic anhydrase inhibitors, age 65 or older, iodinated contrast, surgery with restricted intake, hypoxic states such as acute heart failure, excessive alcohol, and hepatic impairment sit on the list.

Alcohol as a pour question is the alcohol scan. Contrast hours are the dye scan. This page only shows they feed the same box.

What to do if acidosis is the working idea

Discontinue Glucophage or Glucophage XR and use general supportive care in a hospital. Hemodialysis has been used to remove metformin and correct the acidosis in reported cases. That is inpatient work.

Do not 'push through' 850 mg because you do not want to miss a dose while you cannot breathe. Missed-dose politeness does not apply to this picture.

No. It is rare and tied to the risk list. eGFR and sick-day holds are how clinics keep it rare.

Headache was a common-table event. Acidosis is the rare boxed picture with that sick cluster. If you only have a nuisance headache and you look well, that is not this scan.
