Does an occasional nitroglycerin spray still count?
Yes. Regular and intermittent use both sit in the contraindication. A spray 'only if I need it' is still a nitric oxide donor. Hide it on a telehealth form and the fill is unsafe.
Poppers (alkyl nitrites) are in the same wall. They are not a party exception to the 25-100 mg card.
Is an alpha-blocker the same stop?
No. Alpha-blockers need a stable background and a 25 mg sildenafil start. They are not a nitrate-class contraindication. Keep that on the alpha-blocker scan.
Do not confuse a tamsulosin caution with a nitroglycerin ban. The ban is the nitrate.
What if chest pain starts after 50 mg?
Emergency care. Tell the crew the drug name, the milligram, and the swallow time. Do not treat yourself with a nitrate from the drawer.
Stable angina exercise work on the label used men not on chronic oral nitrates. That study is not a license to keep a spray next to 50 mg.
What are the 24-hour plasma numbers after 100 mg?
About 2 ng/mL at 24 hours versus a peak near 440 ng/mL in healthy volunteers after a single 100 mg dose. The label still will not time a nitrate against those numbers.
In people over 65, with hepatic impairment, with CrCl under 30 mL/min, or on erythromycin or a strong CYP3A4 inhibitor, 24-hour levels have been 3 to 8 times higher than in healthy volunteers.
When can a nitrate be given after the tablet?
The US label says that interval has not been determined. Lower plasma at 24 hours is not permission to mix. 'Unknown' is not a workaround.
If chest pain starts after a swallow, call emergency services and say you took sildenafil. Do not take the home spray.
Does 25 mg punch a hole in the nitrate wall?
No. 25, 50, and 100 mg all sit behind the same contraindication. A low rung is not a loophole. The 25 mg start scan is for age, liver, kidney, and CYP, not for nitrates.
Nitrate stop opens the sildenafil panel for a reason. Scan nitrates first.
