The interaction study did not write a zero-alcohol rule. It wrote a 50 mg / 0.5 g/kg / 0.08% result. Zero alcohol is not required. Zero judgment is required when a third pour wrecks the night.
Grapefruit is a CYP3A4 story, not a drink count. Skip the juice the same evening you dose if you want a clean trial.
Why does the board still talk about standing pressure?
Sildenafil and ethanol are both vasodilators. A large alcohol load adds dizziness and a sloppy erection to the same evening even when the formal 50 mg study did not show extra hypotension.
Flush and headache after wine get blamed on 50 mg when the pour already stacked. One or two standard drinks is the usual counseling band this board uses. A third pour is a self-inflicted rating.
