Daily 2.5-5 mg can leave a Tuesday version
Once-daily exposure does not give the 48-hour fade a clean weekend off. Some men accept a dull weekday ache for spontaneity. Others want off-days and leave this lane. Neither choice is moral.
In 12-week daily-use tables, back pain sat at 3% on 2.5 mg and 3% on 5 mg versus 1% on placebo. Myalgia sat at 2% on both daily strengths versus 1%. Those are table rows, not a promise you will or will not ache.
Do not treat the ache with a second PDE5
A date-night extra tablet is not analgesia. It is another exposure. If Tuesday pain is the whole review, the weekend contrast may be the honest next talk, not a 5-to-homemade-8 climb.
Nitrates stay banned while you wait out an ache. So does riociguat. Pain plus chest pressure is emergency care.
What this pattern is not
Sudden one-sided flank pain, blood in the urine, or fever is not Cialis myalgia. Chest pain is not myalgia. A calf that swells after a flight is not myalgia. Those visits do not wait on a 48-hour fade.
Men with moderate renal impairment had back pain as a limiting event at 10 mg in a small pharmacology study. Daily use is already off when clearance sits under 30. Bring the number if the ache is the excuse to ignore the lab.
Forty-eight hours is the usual fade
Typical resolution sat inside 48 hours. When treatment was needed, acetaminophen or an NSAID usually sufficed. A small share needed a mild narcotic in those trials. About 0.5% of on-demand subjects stopped because of this pair.
Pain that climbs after 48 hours, or that arrives with weakness or fever, is a different examination. Do not swallow a second daily tablet to 'push through' an ache.
The 12-to-24-hour start the label actually wrote
Clinical pharmacology trials placed most tadalafil back pain or myalgia in the 12-to-24-hour band after a swallow. The discomfort was often bilateral lower lumbar, gluteal, thigh, or thoracolumbar muscle ache, worse when lying still.
Most reports were mild or moderate. Severe back pain was uncommon among the reports. This is not 'kidney damage' on the Cialis label. The tadalafil panel keeps the same clock.
