Can you take enalapril 20 mg in pregnancy?

Last reviewed · 3 min read · Updated

Bone review card, cobalt edge, lime chip: boxed stop ribbon

No. Pregnancy plus enalapril is a stop. Vasotec carries a boxed warning: when pregnancy is detected, discontinue as soon as possible. Drugs that act on the renin-angiotensin system can injure or kill a developing fetus, especially in the second and third trimesters. Hypernil is an export name for the same INN. There is no taper conversation for later on this card.

What the box actually orders

Discontinue VASOTEC as soon as pregnancy is detected. That is the opening of the boxed warning, not a footnote. A home test that turns positive ends the 20 mg tablet the same day you tell the clinician.

Anyone who might conceive needs the stop decided before a late period. 'We will stop next month if it happens' is how second trimesters get exposed. The panel repeats this stop because it is easy to bury under a cough thread.

What later exposure has done

Use in the second and third trimesters can cut fetal kidney function and bring oligohydramnios. That oligohydramnios has been tied to lung hypoplasia and skeletal deformation. Newborns have shown skull hypoplasia, anuria, hypotension, and renal failure.

Those are labelled fetal-toxicity facts. They are not a reason to stay on 20 mg 'until the anatomy scan.' Stop, then let obstetrics pick a pressure plan that is not an ACE.

This card will not call the first month fine

The box does not offer a first-trimester free pass. When pregnancy is detected, the drug stops. Do not finish a Hypernil strip because you are 'only five weeks.'

If you already swallowed 20 mg after a late period, say so. Bring the last dose hour. Do not add a second ACE from a drawer while you wait for a callback.

Breastfeeding is not this boxed stop

This scan's job is pregnancy. Milk levels and infant exposure are a separate counselling visit. Do not treat a lactation question as permission to stay on 20 mg through a positive test.

If you are not pregnant and the ACE is still indicated, keep the written plan until someone changes it. The OTC scan is still a no.

Hypernil does not rewrite the warning

Export art can look like a different product. The INN is enalapril. The boxed fetal warning travels with the INN. A US generic 20 mg vial carries the same stop.

See whether a US fill is Hypernil if the carton confused a pregnancy plan. Chart enalapril on the obstetric list.

What replaces the ACE is not this page

Obstetrics and the original prescriber pick the next antihypertensive. This board will not name a home substitute from a family drawer. Do not start an ARB on your own the afternoon of the test without that call.

Potassium and creatinine still get a home after the stop if another renin drug is coming. The renal-lane signal is the lab card, not a pregnancy workaround.

Board thread

Answers follow the U.S. Vasotec (enalapril) label.

Mira Landau Is a smaller tablet safer in pregnancy?

Board reply

No. The warning is the class and the renin system, not the milligram. 2.5 mg is not a pregnancy dose on this label.

Cate Bloom The test is positive this morning. Do I take tonight's 20 mg?

Board reply

No. The box says discontinue as soon as pregnancy is detected. Call the prescriber today. Do not taper through the week.

Jules Okeke We are trying to conceive. Stay on Hypernil until then?

Board reply

Ask for a plan now. Waiting for a positive test is how early exposure happens. This scan will not bless staying on 20 mg through unprotected cycles.
